JOB DESCRIPTION: Nurse Health Educator

(Full-Time; 37.5 hours/week; 50% Grant Funded)

Position Overview:

The Nurse Educator (FT) is a member of the KidneyMobile® team. The KidneyMobile is a mobile screening and educational vehicle that travels throughout Illinois screening for kidney disease, diabetes, and high blood pressure. The Nurse Educator must be willing to travel within Illinois 10-25% of the time and will be required to work some weekends. The Nurse Educator should possess at least 3 years’ experience in developing health education programs and the ability to determine educational needs and be able to meet with patients and explain the wellness resources that are available to them. The Nurse Educator will conduct research and develop presentations to address the needs of the community and will also teach patients about lifestyle changes that can help them meet health goals and make wellness an important part of their daily life.

Responsibilities:

​

  • Plan, develop, and design health education and training programs for the KidneyMobile®, including preparation, facilitation, and follow-up.

  • Participate in KidneyMobile screening events and provide on-site health consultations.   

  • Follow-up with KidneyMobile participants to discuss screening results and track outcomes.  

  • Evaluate the KidneyMobile screening program; perform research on relevant topics and develop and/or revise educational and presentation materials as appropriate.

  • Maintain the KidneyMobile inventory spreadsheet and accurately track weekly inventory.  Order supplies with the guidance of KidneyMobile manager.  

  • Present lectures and training to groups and/or individuals about kidney health and risk factors for chronic kidney disease.

  • Prepare and disseminate educational and informational materials. Participate in the development of marketing and promotional goals and strategies to provide education about kidney health and chronic kidney disease.

  • May lead, guide, and train volunteers to perform tasks during KidneyMobile screenings. 

  • Help promote upcoming KidneyMobile screenings. 

  • Represent National Kidney Foundation of Illinois at community outreach events, including health fairs, to provide information and education related to kidney health and chronic kidney disease. 

  • Assist with quality control maintenance on KidneyMobile equipment.

  • Other duties as assigned.


Qualifications:

  • At least three years’ experience developing health education programs; knowledge and experience in adult education principles.

  • Strong attention to detail.

  • Excellent written and oral communication skills; comfort presenting in front of a group.

  • Ability to work well independently or as part of a team.

  • Two to five years’ experience working with patients and/or communities impacted by kidney disease (preferred).

  • Proficiency in Spanish (preferred).

  • Must have a valid driver’s license; carry current insurance on personal vehicle when driven on Foundation business and be insurable to drive for Foundation business.  

  • Must be able to lift 35 lbs

Education:

Bachelor’s degree in nursing or nurse education required.  Must possess current valid license as a registered nurse (RN) or licensed practical nurse (LPN) in Illinois.

Reporting Relationship:

The Nurse Health Educator position reports to the KidneyMobile Manager.
