Bremen Township Wellness Center | Oak Forest, IL

Two urinalysis tests (checking for protein, blood, white blood cells, ketones, dehydration, etc.), blood glucose (sugar), Hemoglobin A1c on all diabetics or if your blood glucose is elevated, body mass index (BMI), waist circumference, blood pressure, and blood draw (people at risk for kidney disease).  After testing is complete, participants will have the opportunity to meet with a healthcare professional (doctor, nurse practitioner and/or nurse) to review their results. Each participant will also receive a printed copy of their test results for their personal records.
