JOB DESCRIPTION: Contract Community Health Worker (CHW)

Maywood & Melrose Park (Full-Time; 37.5 hours/week; grant funded; through 5/31/23)

Position Overview:

The National Kidney Foundation of Illinois is a proud partner of Cook County Department of Public Health’s Building Healthy Communities initiative that advances community solutions for racial and health equity.  The Community Health Worker (CHW) for Building Healthy Communities (BHC) Initiative will work with the Cook County Department of Public Health, PCPs, Local Health Departments, and Community Based Organizations to support clients with kidney disease impacted by COVID-19. The CHW will work closely with local organizations to reach priority populations and build trusting relationships with communities for sustainable, transformative healthy change.  The CHW will work to continue to prevent and control the spread of COVID-19, lessen its health, social and economic impacts, and reduce COVID-19 health disparities --- especially for identified communities and populations most adversely affected by the pandemic in suburban Cook County. This contract position will support NKFI in meeting grant deliverables through town hall meetings, education, and mobile screening.

Responsibilities:

​

  • Engage community partners and identify locations for kidney health screenings in the Maywood and Melrose Park communities.  Schedule and promote the kidney health screenings.

  • CHWs will follow-up with screening participants who had at least one abnormal result and provide information, education, and connect with care services as required. 

  • Schedule and Promote the “Take Charge” program.

  • Schedule and promote Community Health Talks in designated communities.

  • Help connect people to resources as needed, including food pantries, FQHCs, housing, utilities, etc.   

  • Practice social distancing, mask wearing, and always use proper PPE.

  • Able to identify symptoms of distress and act/intervene appropriately.

  • Collects data for the program, being accurate and timely in documentation of client encounters.

  • Follow HIPAA guidelines and maintain HIPAA compliance for clients participating in the BHC program.

  • Participate in all BHC required meetings and trainings.


Qualifications:

  • Proficiency in Spanish (required)

  • Ability to think critically and make decisions based upon available information

  • Must be able to maintain professionalism with clients, coworkers, staff, and referring agencies.

  • Strong attention to detail

  • Excellent written and oral communication skills

  • Ability to work well independently or as part of a team

  • Strong organizational and problem-solving skills

  • Must be self-motivated

  • Must have a valid driver’s license; carry current insurance on personal vehicle when driven on Foundation business and be insurable to drive for Foundation business.

Education:

Associates degree, community health worker training/certificate, or commensurate experience in community health work or social work. 

 

Reporting Relationship:

This contract position reports to the Program Manager.

Position End Date:

This contract position will end on 5/31/2023.
