Topic 12Amgen SAFETY NET®

Amgen's SAFETY NET Program® is part of the Amgen Reimbursement Connection Program which is a patient assistance program, for qualifying patients, that provides access to Amgen products, including Aranesp®, NeulastaTM NEUPOGEN® and EPOGEN® (for dialysis patients only).

Through a simple registration and enrollment process, needy patients who qualify for the program can receive Amgen products through their enrollment provider.

Who Is Eligible? How Can Patients Apply?
Certain uninsured or underinsured patients with limited financial resources are eligible for the Amgen SAFETY NET® Program.

These patients must meet insurance and income eligibility criteria. Patients can apply for assistance through sponsoring health care professionals.

When calling the SAFETY NET® Program, please be prepared with the following information:

  • Provider name
  • Name of contact person
  • Address of contact person
  • Telephone/fax numbers of contact person
  • Patient name
  • Social security number
  • Duration of therapy
  • Patient's annual family income
  • Insurance information for all insurance plans in which the patient participates, including name of insurer, pending insurance coverage and any denied claim (with reason(s) for denial)

HOW TO APPLY

Patients can enroll for this SAFETY NET® Program, through a "sponsoring health care professional" by calling 800-272-9376 .
